Model Truck and Bus Report Fields:
When completing the national review of State crash reports, rather than just identifying common problems and specific deficiencies in the collection of truck and bus data, “model” SAFETYNET data collection elements and attributes were developed.  The “model” was created using the best practices identified in State reports combined with the intent to satisfy all of the ranking criteria developed for the national evaluation.  The report is specifically designed as a modular document to allow for implementation as truck/bus fields on a primary crash report or combined as a stand-alone document. The elements are organized into the groupings of Qualifying Information, Vehicle Information, Motor Carrier Information and other supplementary data.  A PDF of this document can be downloaded below.
